Pay the application fee for a permit to employ a foreign worker in the nursing sector
- The service is intended for people with disabilities according to the National Insurance Institute conditions and have passed a dependency evaluation.
- Permits for foreign workers in the nursing sector are granted in serious cases for persons with disabilities that need assistance with most of their day to day activities, throughout most of the day.
- Persons with disabilities who do not meet the National Insurance Institute conditions for a dependency evaluation because of high income can apply for a dependency evaluation directly from the nursing department.
- If the disabled person cannot meet the conditions to be an employer of a foreign worker, based on age or health, the details of the disabled person's legal guardian or executor who will be responsible for employing the foreign worker must be included on the application.
- The nursing department will reply to the application by post.
- After receiving the permit for employing a foreign worker you must register at one of the registered private employment agencies for foreign workers.

Overview
- You need a permit to employ a foreign worker in the nursing sector.
- You cannot get a permit if you live in a care institution (except for assisted living).
Who can apply
Adults over retirement age
Men over 67 and women over 62 who meet one of the following conditions:
You've taken a National Insurance Institute (NII) dependency test and:
- have 4.5 ADL points
- have 4 ADL points with an additional point
- you're eligible for partial supervision (0.5 a point under the supervision section) and one ADL point
- you're over 90 and have 4 ADL points
If you can't take a NII dependency test high due to high income, you'll be referred to the nursing department to take a dependency test.
Adults between 18 to retirement age
You're eligible for special services by the National Insurance.
Children (under 18)
You're eligible for 100 percent child disability benefits from the NII and meet one of the following conditions.
You:
- need close ongoing special medical care on a daily basis at home. For example, special care needs at night
- need assistance form a nurse throughout most of the day, and cannot attend a school or therapeutic institute
- attend an education institute and are dependent on a nurse to provide physical assistance
IDF veterans and victims of terrorism
You can get a minimum of 17 hours of assisted care if you're eligible according to:
- the Ministry of Defense Assistance Committee
- or the NII Department of Victims of Terrorism with Disabilities.
Job related injuries
You've been recognized by the NII as having a job-related injury and need 'extensive care' or are 'dependent on assistance'.
You might also be eligible if you:
- have a malignant oncological disease and need help for most of the day
- a hospital inpatient soon to be discharged and have both:
- a hospital doctor's diagnosis stating that you need a nurse
- an expected discharge date
- a senior citizen under care of the Ministry of Defense rehabilitation department
Required documents
In all cases you'll need:
- a completed application to employ a foreign worker in the nursing sector
- confirmation of payment of the application fee - ILS 340
- a photocopy of your ID card and slip (of the person who needs a carer)
Required documents depending on the circumstances of the applicant (the person who needs care):
- they cannot fulfill their obligations as an employer - attach a photocopy of the ID card and slip of the person who will be employing the foreign worker for them - close family member, guardian, etc.
- their identity is confidential - provide authorization from their guardian
Children (under 18)
Attach:
- a letter from their kindergarten or school with the hours that they attend
- an up-to-date medical opinion from the treating doctor
- an up-to-date social services report
- the declaration form D
- the NII medical report made when making the claim for child disability benefits
Hospital inpatients soon to be discharged from hospital:
Attach:
- an application for inpatients soon to be discharged (Form E – continued from forms A to C)
- a medical diagnosis from the treating doctor at the hospital/facility stating the need for continuous treatment, and the expected discharge date to the patient's home
The permit will be subject to the decision of the permits department nursing division's representative professional consulting committee, for 3months. This will permit employing a foreign worker already in Israel, and not bringing a foreign worker from abroad)
How to apply
Apply through a private company for foreign workers in the nursing sector
When you apply through a private company that is authorized to act as an agent for foreign workers in the nursing sector, they will handle all the communication regarding the employment with the Population Authority.
This means that you understand and agree that all communication between the company and the authority, will be to the company’s email, registered with the authority, and be considered as legally delivered for all purposed, to the relevant party – you, the patient employing the worker, or relevant family, guardian or the worker.
Apply directly
You - the patient, a close family member or guardian - can apply in one of the following ways:
- by registered mail to Population and Immigration Authority, Permits Division - Nursing Sector, Kich Building, 42 Agrippa Street, floor 5 PO Box 28346, Jerusalem.
- in person to the relevant Nursing Sector Permits Division, during working hours. you cannot apply by fax or email
You will need to provide:
- a home address
- an email for receipt of all correspondence.
Providing your email means that you understand and agree that this email address will be used for all correspondence, including notifications and decisions made by the authority, which will be considered as legally delivered to the relevant party - the patient, family member, guardian, etc.
To apply:
- print the application form
- complete it
- sign it (the patient, family member or guardian)
- submit the original form (not a copy)
When the application is submitted by a private company, it must be kept at the company offices, scanned, digitally signed and submitted by email. The company must also confirm that the original signed documents are held at the company offices for inspection, if required Appendix A).
Make sure to fill out the form clearly and sign it. If the patient cannot sign, the family member or guardian must sign as the 'employer'.
To extend a permit, contact the permits division 60 days before it expires.
Application submitted without confirmation of payment will be returned

Overview
- To extend a foreign worker permit, the person receiving care must complete and submit the application with the relevant required documents.
- The application will be assessed and a decision made.
- If the person receiving care is:
- 85 or over when applying they do not need to be assessed again if their circumstances have not changed (for instance, moving to a care home).
- under 18 when applying, they not need to be assessed again if the meet both of the following conditions:
- they are eligible for a disabled child benefit of 100 percent
- you have attached a declaration that the child does not live in a formal institution
Who is eligible
Adults who have reached retirement age and meet one of the following conditions:
- have 4.5 ADL (Activities of Daily Living) points from a National Insurance Institute dependency evaluation.
- have a 4 ADL points with an additional point from a National Insurance Institute dependency evaluation.
- are over 90 and have 4 ADL points from a National Insurance Institute dependency evaluation.
- are eligible for partial supervision (0.5 a point under the supervision section) and one (1) ADL point from a National Insurance Institute dependency evaluation.
- you have a level 3 nursing benefit or above according to the NII and with a qualifying score as mentioned above
- if you are not eligible for a nursing benefit and have taken a dependency assessment in accordance with Procedure 9.2.0005, your permit will be extended in accordance with this eligibility as long as your circumstances haven't changed, such as moving to an institutional setting.
Adults over 85
Permits previously issued based on the decision of the Professional Advisory Committee permanently or for a maximum period, will be extended, if your circumstances have not changed.
- Adults from 18 to retirement age is someone who has been found eligible for a special services allowance by the NII
Applications to extend a permit for minors
If the permit was given to a minor (under 18) for a 'maximum period', it will be extended without assessment if:
- the minor patient is eligible for a disabled child benefit of at least 100 percent
- the minor does not live in an institutional setting
IDF veterans who were injured in service, and victims of terrorism with disabilities
are eligible for a minimum of 17 hours of assisted care. They must be found eligible by the Ministry of Defense Assistance Committee or the National Insurance Institute Department of Victims of Terrorism with Disabilities.
Job related injuries
If you are recognized by the NII as a disabled worker at the level of 'multiple assistance' or 'dependent on another'
You might also be eligible if you are:
You suffer from a malignant disease or a serious illness and need help for most of the day.
You are a hospital inpatient about to be discharged from the hospital / institutional setting and
the attending doctor has stated that you need the assistance and given a discharge date to your home.

Download, print and complete the following three forms and attach them to your application:
- Form A – foreign worker permit request
- Form B – employment commitment
- Form C – medical confidentiality waiver declaration
- Form D - statement
- confirmation of payment of the reduced application fee - ILS 340
- a photocopy of the person receiving care's ID and slip
Required documents according to the type of application:
- if the person in need of care cannot fulfill their obligations as an employer of a foreign worker, attach a photocopy of the employer's ID card and ID slip (a close family member, or guardian)
- if the person in need of care's identity is confidential you must provide authorization from their guardian
- for children under 18 attach:
- an up-to-date medical opinion from the attending doctor supporting the request, if required during the hearing in the first committee in order to
- statement form D soon to be discharged from hospital you must attach:
- for inpatients soon to be discharged:
- an application form for inpatients soon to be discharged (Form E – continued from forms A to C).
- a medical diagnosis from the treating physician at the treating hospital stating the need for continuous treatment and the expected discharge date (the permit will be subject to the decision of the permits department nursing division's representative professional consulting committee, for a period of three months. This will permit employing a foreign worker already in Israel, and not bringing a foreign worker from abroad)
- to extend a temporary permit granted to a patient discharged from a hospital / institutional setting, attach a letter of release from the last institutional setting you were a patient of before being discharged to your home

Overview
You can freeze (put on hold) a permit to employ a foreign worker in the nursing sector.
You must state on the application that you understand that:
- you can only add care hours according to the National Insurance law after freezing the permit.
- it is illegal to employ a foreign worker while the permit is frozen. You must declare that you will not employ a foreign worker during this period.
- during the 'frozen' period, the permit will continue to be valid. You can apply to cancel the freeze.
- details of the freeze will be passed on to the National Insurance Institute, the Population and Immigration Authority, and the payments department.
- you will not be refunded the application fee for a permit to employ a foreign worker, when you freeze it.
- if the permit period ends while the permit is frozen you can extend it in the same way you would extend an active permit. You will need to pay the permit extension fee.

Overview
You can cancel a frozen permit to employ a foreign worker in the nursing sector.
You must state on the application that you understand that:
- details of your application will also be submitted to the National Insurance Institute. The person in need of care will not be eligible for an additional care pension, according to the National Insurance law.
- the application notice date will be from the day the commissioner gives written authorization.
- you can only cancel a frozen permit if the original permit has not expired. If the permit has expired, you will be notified by the commissioner instructing you to extend the permit, according to the normal procedure.
- the person in need of care declares that they do not reside in a nursing institute (senior citizens home, hospital or rehabilitation center, except for assisted living residence or outpatient care).

Overview
You can a apply for refunds on payments made for a permit for a foreign worker in the nursing sector and, or refunds for payments made for a dependency evaluation.
You can get a refund if:
- you applied to cancel a permit before it was granted.
- the person in need of care died before the permit was granted.
- the person in need of care paid to extend a permit but died before the original permit expired.
- the person in need of care paid twice for the same permit application.
- a dependency evaluation was not made.

Overview
- You can use this service if you need a home care nurse but are not eligible to receive a dependency evaluation from the National Insurance Institute because your income is over the maximum for eligibility.
- You must apply for a permit to employ a foreign worker in the nursing sector before applying for the dependency evaluation.
- You can apply for yourself or someone can apply on your behalf.
- After applying, representatives from the unit will contact you by phone to schedule a dependency evaluation.
What you need
If you do not speak Hebrew, we recommend getting help with the application from a Hebrew speaker.
All documents must be original or copies that have been certified by a lawyer or accountant.
- Your medical information report, completed by a physician.
- Your inpatient medical report if you were a hospital inpatient during the three months before applying.
- A photocopy of your ID (of the person who will be taking the dependency evaluation).
- Confirmation that you have paid the application fee – 340 NIS. You do not need to pay if:
- you are over 85 or under 65.
- you have recently been discharged from hospital or have full National Insurance points.
- you have paid in the past.
- your aaplication has been transferred to an authority ministerial comittee.

Overveiw
- A foreign worker in the nursing sector can use this service to extend their permit.
- The application must be made by one of the private foreign worker agencies.

Pay the reduced fee for a foreign worker in the nursing sector permit – NIS 340
- The payment is non refundable (even if the permit is not granted).
- You cannot get a reduction for this application fee, including requests for a reduction based on low income.
Pay for a dependency evaluation – NIS 330
For persons over retirement age who are not eligible for a nursing pension based on income, and need to take a dependency evaluation.
Pay to extend a permit for a foreign worker in the nursing sector – NIS 195
When two persons in need of care, living in the same household, submit an irregular joint request to employ one foreign worker, you only need to pay once, unless you need two dependency evaluations.
Employers who have received apermit to employ a foreign worker must arrange the foreign worker's work permit. Work licence (1/b) will be stamped on the foreign worker's passport stating the sector they are licensed to work in.
