Use this service to choose your last name that will be registered in the population registry, after getting married .
Choosing your last name after getting married
After getting married, you can choose your last name from one of the following options:
- Keep your current last name.
- Take your spouse's last name.
- Keep your last name and your spouses last name in either order.
- Choose a new last name for both of you.
- Choose a new last name for both of you, and add it to each of your last names.

What you need
- Your ID
- Your spouse's ID appendix
- Both your Israeli passports
- Your original marriage certificate. If your marriage certificate was granted abroad:
- you must get it translated by a notary to Hebrew. You don't need to translate and notarize marriage certificates in English or Arabic
- it must be officially authenticated with an apostille stamp. For certificates without an apostille stamp, contact your nearest Israeli mission in the country where the certificate was issued for instructions
Service fee
- Free for one application per applicant within 6 months after registering your marriage.
- All additional applications cost 125 NIS

How to apply
In Israel
- Download, print and fill out the Notification of choice of last name form
- Make an appointment at an authority office
- Submit the application in person, together with the required documents at your local Population and Immigration Authority office
After you change your name you will need to a issue a new ID card and passport. If your passport is valid for another year or more, you will not have to pay for the new one.
Abroad
Apply at an Israeli mission if you are abroad
Notes
- You can change your last name only once every 7 years.
- In some circumstances, you will need to present Israeli passports for all the family (for children over 16 years old).
- If you don’t speak Hebrew we recommend asking a Hebrew speaker for help with forms and applications.
