Request a new ID appendix following changes in your personal information
Apply to change details on your ID appendix, following changes in personal information such as marriage, divorce or the birth of a child.
Who can apply
- Israeli citizens and residents aged 16 and over.
- You can request the service for yourself or for your spouse.
What you need
- Your ID card.
- The current ID appendix being changed.
- Your Israeli passport and your spouse's Israeli passport if you are making the change on their behalf.
- A public document of record, such as a marriage or divorce certificate, or notification of birth from an Israeli hospital.
The service is free of charge.
How to apply
- Print the ID appendix form.
- Submit the application in person, together with the required documents, to your local Population and Immigration Authority office.
Notes
- If you are interested in changing your name to one that is not registered on a marriage certificate, you must do so in person and complete and submit the Change of family name form.
- This service and forms are provided in Hebrew. If you don't speak Hebrew, seeking assistance from a Hebrew speaker is recommended.
