First degree relatives can use this service to request a death certificate of deceased family members who passed away in Israel.
Who can issue a digitally signed death certificate
First degree relatives of the deceased (spouses, a parent for his/her children, children for their parents).
How can I issue digitally signed death certificate
1. You can download a digitally signed death certificate through the government's online personal area.
2. To enter the personal area, you must register to the Government identification system.
3. Under section “documents” you will find the digitally signed death certificate of the deceased that you can download to your computer or cell phone.
Who can apply for a death certificate through an online application form
Spouses, children, parents of the deceased, or interested persons (on submission of a letter of explanation) can request a death certificate of persons who have passed away in Israel.

What you need
- The ID number of the deceased.
- Personal details of the deceased (you must state the original details as recorded at the time of death).
- If you are not a first degree relative of the deceased, you must submit the application in person at your local Population and Immigration Authority office together with a letter detailing the reasons for the request, or power-of-attorney from a relative of the deceased.
How to submit the request
Complete the online death certificate application form and submit it online, or print the form, complete it by hand and submit it to your local Population and Immigration Authority office.
The service is free of charge.
Notes
- It is important to provide your contact details in case of missing or incorrect data on your application, so that an authority clerk can contact you.
- The death certificate will be sent within five work days of submitting the application to your address as it appears in the population registry.
- In cases in which the deceased passed away abroad, you can request a population registry extract instead of a death certificate.
- This service and forms are provided in Hebrew and English, but you can only enter details in Hebrew. If you don't speak Hebrew, seeking assistance from a Hebrew speaker is recommended.

Self-service stands
You can submit the application at one of the self-service stands that can be found at Population and Immigration Authority offices.
Service box
You can place the printed application in the service box, found next to the self-service stands in the authority offices during office work hours, or outside the authority offices, 24 hours.
Office
You can submit the application in person at your local Population and Immigration Authority offices.
Post
You can submit the application by post to: The National Center for Service and Information, 42 Agripas Street, Jerusalem, Israel.
Online
You can complete and submit the death certificate application form online.
