Submit a multi-annual report
Use this service to open a collection file at the National Insurance Institute, and to make notifications of changes, in order to set the correct level of national insurance payments, and benefits that you are entitled to recieve.
After submitting the report, the National Insurance Institute will register your correct status for national insurance payments, claims and benefits, and ensure that you receive your rights.
What you need
- If you are self-employed, you must complete and submit the Power of Attorney Representative form.
- If you are an employee, you must have your last 12 salary slips or 106 form.
- If you are a housekeeper or an employee currently on unpaid leave, you must have a confirmation letter from your employer.
- Kibbutz members must have a kibbutz membership confirmation letter.
- Pensioners must have a pension slip or pension confirmation letter from the pension payer.
- Prisoners and detainees must have a letter of confirmation form the prison, or community service officer, or court ruling for house arrest.
- Yeshiva students must have a confirmation letter from the yeshiva.
- If you are enrolled in professional training you must have a confirmation letter from the body supervising the training.
The service is free of charge
Guidelines for attaching the required documents:
When uploading more than one document, it is important to try to reduce the overall upload size of the documents.
The maximum single document upload size is 500KB
The maximum combined uploaded documents size is 10MB
Scanned documents must be submitted in black and white, and at a resolution of up to 300 DPI.
Password protected documents are not accepted.

Fax
Submit the completed Multi-annual report form and required documents to the Payments Department for Non-employees by fax to your local National Insurance Institute office. Write your ID number, clearly, at the top or bottom of each faxed document.

Submit the completed Multi-annual report form and required documents in a service box at your local National Insurance Institute office.
