Retrieve a vaccine administration record (vaccination booklet)
You can reproduce a vaccine administration record (vaccination booklet) for yourself or your children.
The vaccine administration record is proof of the vaccinations you’ve received. Under the Archives Law, children's medical records are kept until the age of 25, after which retrieving the vaccination history may not be possible.
Who can restore a vaccination booklet?
Parents of children and adolescents can restore a vaccination record for their children. The service is also available to adults under 25.
The service comes with a fee
The current fee can be found in the service fee schedule (Hebrew, Code L0268).
Payment must be made in advance.
If the request is at a family care center, it is via Israel Post, using a payment slip given by the family center.
If the request is at the health bureau, payment is made at the bureau.
If the request is via Student Health Care, payment is made to the Student Health Care providers.
How to Submit a Request
For children up to age 16
Your child’s vaccination record can be found in the digital vaccination record in the government’s personal area or in the personal area of the health maintenance organization (HMO) where they are insured.
For adolescents and adults aged 16 to 25
It’s best to first check the digital vaccination record in the government’s personal area or in the personal area of your HMO. If the information is missing or incorrect, you can reach out to the Health Bureau or Family Care Center (Tipat Halav):
Contact the health bureau (Hebrew) by phone or fax in the area where you received the vaccine. Restoration is only available for vaccines given at the Family Care Center (Tipat Halav) or Student Health Care.
In Jerusalem and Tel Aviv, Family Care Center (Tipat Halav) services are run by the municipalities. If you were vaccinated in these cities as a child, contact public health services:

For adults over age 25
If you were vaccinated at a Ministry of Health Family Care Center (Tipat Halav), you can try to retrieve your vaccination record at the Health Bureau in the area where you received the vaccine
