Set disability percentages for tax exemption - Form 169
You can apply to the Tax Authority to set disability percentages for tax exemption. The disability percentages are set by a National Insurance Institute medical committee.
Who can apply?
Those who are interested in receiving a medical disability percentage for income tax exemption.
The disability percentage is set by a National Insurance Institute medical committee.
Those who have already been determined to have a medical disability that entitles them to tax relief can activate them as detailed:
- Income tax relief - through the preparation of tax coordination with the committee's decision and the protocol.
- Purchase tax relief - by applying for partial exemption from purchase tax (form 2973) while submitting the purchase statement.
What you need
- The payment voucher for the National Insurance Institute.
- Medical documents relevant to your disability.
Payment and costs
679 NIS for the first commission.
Application fees may be updated from time to time.
The committee's expenses must be paid in one of the following ways:
- The National Insurance website – online.
- Postal bank voucher.
- Bank voucher
You can get the voucher at the National Insurance or Income Tax offices, during reception hours.
How to apply
- Apply Online - you must log in to the Medical Committees system for tax exemption.
- Manual submission - Print and complete the application to set disability percentages for of tax exemption. Fill it out according to the instructions on the form. The application submission date will be determined as the date on which all the documents, the full Form 169, and the payment of the committee's expenses were submitted. Attach the required documents through one of the following channels:
- Online Public Inquiries System.
- Postal delivery - to the income tax office nearest to the applicant's place of residence.
- Office - in person to your local Tax Authority office. It won't be possible to receive an audience without an appointment. To make an appointment at the Tax Authority offices.
What happens next?
- Your application will be forwarded to the medical committee of the National Insurance Institute to determine your medical disability percentage.
- The National Insurance Institute will notify you of their decision and update the Tax Authority.
- The Tax Authority will update the decision on their system and notify you.
How to appeal a decision
- You can appeal a decision within 45 days of receiving the notification from the Tax Authority
- It costs ILS 979 to appeal.
