Application for Students Pursuing Healthcare Professions to Verify Immunization Schedule Completion
This service allows students of healthcare professions to submit an online application for verification of completion of the immunization schedule and required testing.
Healthcare and medical students are required to complete their immunization schedule and to perform certain tests in order to receive their "Form C" that attests of getting vaccinated.
Who may file the request?
Healthcare and medicine students.
What do they need to attach?
- Photocopy of their identity document, including the slip.
- Photocopy of their vaccine administration record (vaccination booklet) or any other form of documentation attesting to the administration of vaccines.
They may also have a printout of their vaccinations from their digital vaccination record. However, the vaccinations in the digital record are only updated from 15.3.2025, and therefore this printout should be attached to the paper vaccination record (and not instead of it).
- In certain healthcare professions, the following documents are also required:
- The results of the tuberculin skin test (Mantoux test): For students who are required to take the minimum track or the basic track.
- Results of an HBV test: For students who are required to take the extended tracks.

How much does it cost?
The service is provided free of charge and includes consultation with a doctor.
How do I file the request?
You may file the request in one of the following ways:
- Online: You need to fill out the online form and enter all the details and attach all of the required documents in a clear and legible manner.
- In the health bureau or the HMO clinic: You may also show up physically, upon scheduling an appointment, to one of the vaccination clinics in the health bureaus or in the HMOs, and there file an application to receive your Form C.
Continued processing of the request
- The nursing staff in the Ministry of Health's Call Habriut hotline will examine (according to the information received from the request form) if the students have completed all of their required vaccinations and tests in accordance with the Vaccination of Healthcare Students and Employees Circular (Hebrew) and will issue a certificate attesting to the completion of the immunization schedule—Form C. This certificate will be sent to applicants by email.
- If students need to catch up on vaccinations or tests, they must do so at the health bureaus, vaccination clinics, HMO clinics, or private clinics.
