Consumption Follow-up Table for Patients Treated with Medical Cannabis
This consumption follow-up table is designed to monitor the patient's actual cannabis consumption, in order to ensure that the cannabis is consumed in an optimal manner.
Who needs to fill the consumption follow-up table?
- Patients for whom a request has been filed to change the dosage to more than 40 grams per month are required to fill the consumption follow-up table and to attach it to the request to increase their dosage.
- The IMCA physician who reviews such license requests may require the inclusion of such a consumption table to the license request.
How do I fill the table?
First you need to download the file to your computer. The file is in doc. format and you can either fill it in on Microsoft Word or print it and fill it manually.
In each line you need to provide consumption date, manner of administration (smoking, vaping, oil and more), consumption time, consumed amount, reasons for consumption and a ranking of the drug's effect on a scale of 1 to 10, when 1 = had no effect whatsoever and 10 = very effective.
Patients will fill the data to reflect actual consumption.
- Once the physician submits the request to increase the dosage, the patient will receive an e-mail with a link to the request. In this link it will be possible to send this table as well as additional documents that are necessary for the completion of the request.
