Submission Confirmation System (Form 126/856)

To confirm the declaration submission data, log in to the Submission Confirmation System – Form 126/856 on the Tax Authority website or through the representative portal on a computer connected to the Tax Authority System (Sha'am) using a smart card issued by an authorizing authority.

Enter the deductions file number, tax year, form type and barcode number that appears on the upper left side of the declaration form and confirm the submission data by clicking the "Confirm" button.

For declarations submitted for the years 2014-2018, the confirmation of submission data can be done by choosing one of the following alternatives:

• Print the declaration form, have the form signed by the employer/withholding agent or representative and send it for reception at the Deductions Center in the Operations Department via email [email protected], or by mail to P.O. Box 2648, Beit Daniel, Jerusalem, 91026.
  • Alternatively, confirmation of the deduction submission data can be done online using the Submission Confirmation System – Form 126/856 on the Tax Authority website online or through the representative portal on a computer connected to the Tax Authority System.

Declarations submitted for the years 2008-2013 can be confirmed by printing the declaration form, having the form signed by the employer/withholding agent or the representative and sending it for reception at the Deductions Center in the Operations Department via email [email protected], or by mail to P.O. Box 2648, Beit Daniel, Jerusalem, 91026.

Regarding declarations submitted for years up to and including 2008, Part A of Declaration 0856, which was produced by the reporting software of the declarant, must be attached. Starting with declarations from 2009 onwards, Part A does not need to be attached to the declaration.

• It is recommended to keep copies of the declaration form and the appendices.
• As a reminder, a declaration will be considered as submitted only after the declaration form has been sent and received as required.
• The date of submission the declaration is the date on which the declaration form and all appendices were received at the Deductions Center or the declaration data was confirmed online.
