Parental Consent Form for the Participation of Youth Aged 15-18 in Performances, Modeling, or Advertising and Mediation for Employment in These Performances (Representation by Agencies)
This service allows parents of teenagers to submit a form to the Ministry of Labor, which will also be sent to employers or intermediaries, granting consent for their child's participation in modeling or advertising performances and approving their representation by an intermediary (agencies).

- This service is an initial implementation of the "Ask Once" policy to simplify the process of receiving online services.
- After the national personal identification process, some of the required information for processing the request will be automatically entered.
- At this stage, the information that will be entered is:
- Parent details.
- Children's details.
- Contact details.
- In the future, the service will be expanded to include income and benefit data.
- The service is intended to enhance the protection of youth aged 15-18 in employment and mediation for employment in performances, advertising, including modeling.
- It is important to ensure that parents provide their consent for the mediation and employment of their sons and daughters.
The parent or guardian of teenagers aged 15-18 in one of the following situations:
- Who are interested in mediation for performances and advertising (e.g., representation by an agency), including modeling.
- Who are interested in being employed in such performances.
- A copy of the parent's (or guardian's) ID card.
- The appendix of the parent's ID card, including the details of the teenager for whom the form is submitted (or a document confirming the appointment as a guardian).
- Employer details (name, private company number or licensed business number).
- The written consent of the teenager.
Identification of the applicant
- Before filling out the application form, a quick, simple, and secure identification process is carried out through the national identification system, which includes a connection to government databases.
- Logging into the national identification system simplifies the registration process, exempts parents from entering information already in the system, and eliminates the need to submit some documents.
- If you are registered in the national identification system, you will be asked to log in using a password and a one-time code that will be sent to you.

Submitting the application
The parent and teenager's details must be filled in:
- Parent's details (first name, last name, ID number, copy of ID card, copy of appendix, selection of family relationship, mobile phone number, email address, residential address).
- Teenager's details – first name, last name, ID number, gender, and email address.
Filling in the employer's or intermediary's (agent's) details.
The following details must be filled in:
- Company identifier type – private company (registration number) or licensed business.
- Company name, company phone number, email address, city, street, house number, and contact person details from the company (last name, first name, ID number, phone number).
First page – Employment details and declaration
The following details must be filled in:
- Nature of the approval:
- Mediation for the employment of youth in advertising performances through an intermediary.
- Representation of youth in advertising performances with the employer and a description of the event.
- Parental declaration – including personal details (first name, last name, ID number).
- Consent of the teenager (first name, last name, and ID number).
According to the law, employers and intermediaries must keep a record of this consent for seven years.
After submitting the form, parents and employers or intermediaries (agents) will receive the consent form at the email address provided in the form.
