Report eyesight test results for drivers’ licenses
Licensing Authority optometrists can update the results of eyesight test results online, for drivers’ licenses. You’ll need a smartcard to use the service.
To report eyesight test results:
- connect your smartcard to the card reader
- enter the online report
- fill out the details of the applicant (the person who received the eyesight test), and the license grade. If they are applying for more than one license, the eyesight test must be according to the most stringent standard of the two
- fill out the test results – pass or fail
- fill out the statement that you:
- recognized the applicant with their ID
- performed the test in person
- submit the report
- print a copy of the test form. A copy should be given to the applicant, even if they didn’t ask for one
