Issue a notification of death and a burial permit
This service provides information on the process of issuing a notification of death and a burial permit
A notification of death is a document issued by a physician to confirm the death of a person. It is required to receive a burial permit.
Note: a notification of death is not a death certificate. A notification of death is issued by a physician while a death certificate is issued by the Population and Immigration Authority for official and legal purposes.
The service is provided free of charge
How to issue a burial permit
- If the death occurs in a hospital
A physicn at the hospital will fill out a notification of the death and give it to the family or the admission and discharge office. - If the death occurs outside a hospital
If a physician with a license issued by the Ministry of Health is present, the physician will announce the death and fill out a notification of death.
If there is no physician presernt, call a Magen David Adom intensive-care ambulance (or another licensed ambulance service provider). A paramedic will announce the death and fill out a notification of death.
Police officers may also be called, as determined by the medical team.
The paramedic or doctor who was present will fill out the notification of death. It will be passed on to a health bureau where it will be signed and completed.
How to get a burial permit
It is prohibited to bury a person without a burial permit.
A burial permit is given in the following ways:
- If the death occurs in a hospital that issues burial permits - at the admission and discharge office.
- If the death occurs in a hopital that does not issue death burial permits or outside a hospital - from a health bureau, by presenting the following documents:
- notification of the death filled out by the medical team that was present when the death was announced - 4 copies
- report by Magen David Adom (if a team of the organization was present)
- notification of the death filled out by a paramedic of Magen David Adom (if a paramedic of the organization was present)
- waiver of autopsy form signed by the Police (if called to the place of death)
- an official identity document with a photo of the deceased, for example, their ID or passport
- you should also submit documents regarding the health of the deceased before the death, as well as contact information and the name of the attending physician
Notes
- If the health bureaus have closed for the day and only if you wish to lay the deceased to rest that same day, call the Burial Society directly. The Burial Society will coordinate obtaining the burial permit with a physician at the health bureau.
- Some hospitals that issue burial permits give the information directly to the Population and Immigration Authority. You can check with the hospital.
