Appendix for the Application to Treat PTSD Patients with Medical Cannabis
Those who request a license for medical cannabis and who are diagnosed with Post-Traumatic Stress Disorder (PTSD) are required to attach to their request this appendix for PTSD patients.
Who fills this appendix?
The appendix to the application for the treatment of PTSD patients with medical cannabis is filled by the treating psychiatrist of a PTSD patient who requests a license for possession and use of medical cannabis.
When should the appendix be filled?
The appendix should be filled after the treating physician, who recommended the treatment with medical cannabis, filed the application for the license for possession and use of medical cannabis.
This appendix will only be filed in the initial request for that patient. There is no need to submit it with each license renewal request.
- Once the recommending physician fills the initial request form, the patient will receive an e-mail with a link to the request. In this link it will be possible to send this appendix as well as additional documents that are necessary for the completion of the request.
