Reporting an Aircraft Accident or Incident
This service enables reporting an aircraft accident or incident.
Who is required to file the report
The Aviation Law (Sections 106 and 138) requires anyone involved in a civil aviation safety incident to report it.
To whom should the report be submitted
The report should be made both to the Aviation safety Investigation Authority Israel (IAIA) and to the Civil Aviation Authority (CAA)
How to submit a report
Any civil aviation accident or severe safety incident must immediately be report to the Aviation safety Investigation Authority Israel (IAIA) in the Ministry of Transport. The report must be submitted as follows:

- Landline: 073-3448151 on weekdays, during business hours.
- If you have not succeeded in reporting to the Head of the IAIA, or to the on-call investigator, you must report to the Transportation Call Center at: 03-9545444.
The call center operates on a 24/7 basis.

Online reporting
- In addition, any accident or incident of any kind, even if it has been reported over the phone to the Head of the IAIA or to the on-call investigator, must be reported in writing to the IAIA and to the Civil Aviation Authority within 24 hours. This must be done on an online form entitled notification and reporting an aircraft accident or incident.
