Requesting a Permit to Import Unregistered Medical Products
This service allows you to submit an online application to import unregistered medical products in accordance with Regulation 29.
Who can apply for the import of unregistered medical products?
- A private pharmacy that conducts private import.
- A business or a government pharmacy.
Required application materials (Hebrew)
- Forms and documents outlined in Procedure 33: Import and Marketing of Drugs and Pharmaceutical Substances.
- Pharmacist in Charge Declaration Form Under Procedure 129.
- Notification of administrator's consent (Hebrew), in accordance with regulation 29:
- Form 1: Declaration on import of a Product for Personal Use.
- Form 2: Unregistered Medical Product Approval or Off-Label Use Approval for a Medical Institution Use (Collective Approval).
- Form 3: Physician’s Prescription for an Unregistered Medical Product or a Labeled Medical Product Not Registered in a Medical Institution for an Individual Patient.
- Form 4: Physician's Order for Compassionate Use or Emergency Treatment for an Individual Patient.
- Form 5: Prescription for an Unregistered Medical Product or an Off-Label Medical Product for a Private Pharmacy.
- Form 6: Physician's Order for a Veterinary Product Not Registered in the registry.
- Form 7: Physician's Order for a Veterinary Product Not Registered in the registry, for a Veterinary Hospital.
- Form 8: Veterinarian’s Order for an Off-Label Veterinary Product.
- Form 9: Application Form for Approval of Manufacturing or Use of Advanced Therapy Medicinal Product (ATMP).
The service is free of charge
How to submit the application?
Fill out the application form for issuing unregistered medical products under regulation 29 and submit online along with the required documents.
- You can only submit requests using a computer, not a mobile device.
- The system may open on a mobile device, but it hasn't been designed for that, so it might not work correctly.
- Before using the MASLUL+ system, you need to verify your identity with the national identification system.
- You can verify your identity with a smart card or a username and password.
Further application processing
- If you are required to include additional documents, you will receive a notification by email or SMS.
- To add missing data, submit a new online application.
- Application processing time is 5 business days.
- Upon verification of document validity:
- An import permit will be sent to customs.
- Importers will receive notification by email or SMS.
